Butler, who finished with a game-high 30 points, landed hard on his back midway through the third quarter and headed to the locker room.
He returned to the bench but did not re-enter the game because Miami was already winning by a significant margin.
Head coach Erik Spoelstra said post-game that Butler could've gotten back in the game but wasn't needed.
"We pushed the lead to 20," Spoelstra said. "I was going to insert him back into the game if it broke 15."
The Heat lost guard Tyler Herro in Game 1 after he suffered a broken hand.
Miami sustained another significant setback when Victor Oladipo suffered a knee injury in the fourth quarter of Game 3. The injury was serious enough that Oladipo needed assistance to leave the court.
Miami leads the series 2-1.
